Simply fill out the form below and your name will be included on the Phoenix DVD along with Visions of Mars, a collection of Mars literature and art, and personal messages to the future by space visionaries of our time such as Carl Sagan, Isaac Asimov, Ray Bradbury, and Arthur C. Clarke. .
Don't forget to print your official participation certificate when you are done!
The deadline for inclusion on the DVD is February 1, 2007.
Note to Members of The Planetary Society:
If you are a Member of The Planetary Society as of September 30, 2006, your name has already been included in the list of names to fly to Mars. Your special Fly Your Name to Mars certificate is already waiting for you!
